ELISA (Enzyme-Linked Immunosorbent Assay) Test

The ELISA tests detect antibodies, a type of protein, generated by your immune system in response to the bacterium B. burgdorferi, but if your immune system is weakened then there are chances of a false negative.

Your blood sample is taken to the lab where the specific antigen, toxin, or other foreign substance which induces an immune response in the body is mixed with it. If your blood sample has the antibody, then it binds to the lab-introduced antigen. Then an enzyme is added to the blood sample which changes the color of the blood, provided there is the antibody in your blood sample to bind with the antigen. The change in the blood color indicates the presence of antibodies for B. burgdorferi, but your blood sample may not have the antibody because your immune system is too weak to produce antibodies in response to the bacterium. However, the bacterium may still be present in your blood, but it avoids detection via the ELISA test due to the lack of specific antibodies in your blood sample.

Western Blot Test

If you test positive in the ELISA, then the Western Blot is recommended as a confirmatory test.

Western blot is more helpful for determining the presence or absence of a particular protein than obtaining more detailed information in practice. This qualitative testing process poses a challenge in circumstances where there are weak or muddled protein signals which can make or break an early diagnosis.

In the case of Lyme disease, Western blot is less than ideal for making an early diagnosis because it can’t detect infections until a later stage, and at a later stage we run into the problem of the suppressed immune system causing a lack of detection. Since both ELISA and Western Blot detect the disease based on the immune system’s response to the pathogens, it may be challenging to detect pathogens if your immune system is weakened. Your immune system may be affected if the pathogens have been in your body for some time, or if you have any other physiological or psychological conditions.
